    • you should provide us with your name, surname, address, e-mail address, phone number, relationship to the person for whom you are requesting services; you should provide the name, surname, personal identification number, e-mail address of the person for whom you are requesting services. postal address / phone, municipality, city, full address, how many people need assistance, and also attach: a copy of the personal identity card / passport, copies of all documents demonstrating the need for social services, the content of the inquiry and the information relevant to it. After submitting the inquiry, you may be asked to provide additional information about yourself so that we can properly respond to your inquiry.
